Advisement & Forms

Graduate students are advised to consult the BME Graduate Student Handbook, the UNM Course Catalog, the Office of Graduate Studies website, or speak to the BME program coordinator or Program Director. Commonly requested forms are listed below. Unless otherwise noted, all of the forms are available from the OGS website: http://grad.unm.edu/resources/gs-forms/.  All forms should be submitted to the BME Graduate Program Coordinator, who reviews the forms prior to their submission to OGS.

Forms for Students in the Plan II (Non-thesis) MS Program: 

  • After 12 credit hours have been completed:
    • A “Program of Studies” form must be filled out and submitted to the Office of Graduate Studies. 
  • Two weeks prior to the MS defense:
    • An “Announcement of Exam” form must be filled out and submitted to the Office of Graduate Studies (1st page of this form).
  • At the conclusion of the MS defense:
    • A “Report of Examination” form must be filled out by the faculty and submitted to OGS (2nd page of this form.) NOTE: for Plan II students, the “Evaluation of Thesis/Dissertation Manuscript section (top left) should be left blank.
  • In the student’s final semester, he or she must contact the BME Program Coordinator to ensure that their name is on the list of graduates for the semester.

Forms for Students in the Plan I (Thesis) MS Program:

  • After 12 credit hours have been completed:
    • A “Program of Studies” form must be filled out and submitted to the Office of Graduate Studies. 
  • In the student’s final semester, he or she must contact the BME Program Coordinator to ensure that their name is on the list of graduates for the semester.
  • Two weeks prior to the MS defense:
    • An “Announcement of Exam” form must be filled out and submitted to the Office of Graduate Studies (1st page of this form)
  • At the conclusion of the MS defense:
    • A “Report of Examination” form must be filled out by the faculty.
    • One copy of the “Report on Thesis or Dissertation” must be filled out by each committee member.
  • One copy of the MS Degree Outcomes Assessment Rubric.  This is a School of Engineering form being used to evaluate the success of our graduate program.

Forms for Students in the PhD Program:

Qualifying Exam:

Comprehensive Exam:

  • In their 3rd or 4th year, BME graduate students in the PhD program are expected to complete the Comprehensive (Candidacy) Exam.
  • Two weeks prior to the comprehensive exam, an “Announcement of Exam” form must be filled out and submitted to the Office of Graduate Studies (1st page of this form).

Appointment of Dissertation Committee:

  • This form should be submitted as soon as the Comprehensive Exam has been taken, and when the student begins taking 699 credits (Dissertation credits).
  • PhD committees must consist of a minimum of four members, two of whom must be tenured or tenure track professors affiliated with the BME graduate program. The remaining faculty members must adhere to the specifications in the OGS Guidelines for Appointment of Dissertation Committee. 

Application for Candidacy:

  • This form should be submitted as soon at the same times as the Appointment of Dissertation Committee (above).
  • This form summarizes the program of studies for the degree, including all courses (core, elective, and other) that you will apply toward your degree, including those you intend to enroll in the future.  

PhD Defense:

  • In the student’s final semester, he or she must contact the BME Program Coordinator to ensure that their name is on the list of graduates for the semester.
  • Two weeks prior to the Dissertation defense, an “Announcement of Exam” form must be filled out and submitted to the BME Program Coordinator: (1st page of this form).
  • At the conclusion of the Dissertation Defense:
    • Report of Exam form.

    • One copy for each committee member of the Report on Thesis or Dissertation.
    • One copy of the PhD Degree Outcomes Assessment Rubric.  This is a School of Engineering form being used to evaluate the success of our graduate program, and is available from the BME Program Coordinator.
